AFFIDAVIT

I, NIÑO KIM L. ARONG, of legal age, single, Filipino and resident of Lilo-an,
Cebu, after having been duly sworn in accordance with law, do hereby depose and
state:

1. THAT my complete name is NIÑO KIM L. ARONG;

2. THAT I have been using the first name NIÑO KIM in all my documents, records
and transactions knowing it to be true and correct;

3. THAT in my Birth Certificate issued by the National Statistics Office (now the
Philippine Statistics Authority) the entry for my first name is NINO KIM;

4. THAT the said entry is incorrect and I wish to have the said entry for my first
name rectified and changed to “NIÑO KIM”; and

5. THAT I am executing this affidavit to attest to the truth of the foregoing facts
and for whatever legal purpose it may serve.
