AFFIDAVIT

I, NELIA ABREU GARCIA, of legal age, married, Filipino, and presently

residing of 8 Salinas, Marulas, Valenzuela City, after having been duly sworn to in
accordance with law do hereby deposes and states, that:
 
1. I was born on July 12, 1960, in Manila, and my biological parents are,
namely, CESAR SENO ABREU and JUANITA CAMPOS, who got married
on August 31, 1987, at St. Joseph the Worker Parish, Quezon City, both of
them are now deceased;

2. There was delay of the registration of the fact of my birth with the Civil
Registry of Manila due to inadvertence on the part of my parents. Due to
this, my parents failed to secure a Certified True Copy of my Certificate of
Live Birth;

3. Nevertheless, I have been using the surname of my father “NELIA

CAMPOS ABREU” in all of my transactions such as my school records and
other pertinent documents;

4. On November 11, 2021, I went to the Office of Philippine Statistics

Authority to request a Certified True Copy of my Certificate of Live Birth.
However, I found out that my name was not entered thereof;

5. The Office of Philippine Statistics Authority required me to execute this

instrument in compliance with the requirements FOR ISSUANCE OF A
COPY PSA ANNOTATED BIRTH CERTIFICATE AFTER THE PROCESS
OF LEGIMITATION;

6. In view thereof, I have caused the preparation of this instrument in

compliance with the requirements of the PSA for the process of my
Certificate of Live Birth in connection with the used of surname of my late
father and other related concerns with the Office of Philippine Statistics
Authority; and

7. I am executing this Affidavit to attest to the truth and veracity of the

foregoing set of facts and to inform the authorities for whatever legal
intents and purposes it may serve.
