AFFIDAVIT OF CONSENT FOR CHILDREN TRAVELLING ABROAD

I, NOREEN ABILAY-RAYOS, of legal age, married, Filipino citizen and residing at

__________________________, after being duly sworn in accordance with law, hereby
depose and state that:

1. I am the solo parent of ANGELIKA JUSTINE ABILAY RAYOS, 14 years old and
DYLAN JAMES ABILAY RAMOS, 13 years old who are both under the custody and
guardianship of my parents ALFONSO ABOGADO ABILAY and NORALYN CEA-ABILAY;

2. They have been staying with my parents since I separated with their father whose
whereabouts are unknown, and I had to work in Malaysia for their support;

3. Since I could not travel home to see them more often due to my work, I have
intended for them to come and see me instead during their school vacation;

4. The aforecited planned travel therefore have my full consent since they would be
travelling with my parents for their security;

5. This affidavit is executed to attest to the truthfulness of the abovecited

circumstances, necessary for my minor children’s travel as required under our laws;

6. Further, it is likewise to inform the proper government agencies, and for all other
legal intents and purposes.
