AFFIDAVIT OF DISCREPANCY

The undersigned, , SUSAN PANAL BATTAD, of legal age, Filipino citizen, with residence
address at Fantail St., Area 3 Sitio Veterans, Brgy. Bagong Silangan, Quezon City,after having
been duly sworn to in accordance with law do hereby depose and say:

1. That I am the same SUSAN BOLDIOS PANAL who was born on September 6, 1964,
attached is a copy of my Birth Certificate issued by the National Statistic Office in
Quezon City;

2. That on December 2, 1995 I contracted marriage to Jose P. Battad at Parokya ng

Mabuting Pastol, Quezon City; attached is a copy of our Certificate of Marriage;

3. That I am the mother of JASPER-SHERIDAN P. BATTAD, born on May 31, 1989, in

Marikina City,

4. That however in my son’s Birth Certificate my name was erroneously entered or recorded
therein as SUSAN CUMPA PANAL;

5. That for all legal intents and purposes, the names SUSAN BOLDIOS PANAL
(indicated in my Birth Certificate) and the SUSAN CUMPA PANAL( indicated in
my son’s Birth Certificate) refer to one and the same person none other than the herein
affiant;

6. That I am executing this affidavit to attest to the truth and veracity of the above foregoing
facts and for all other legal and purposes it may serve.
