AFFIDAVIT OF DISCREPANCY

I, JESUS MANUIT ZABALA, of legal age, Filipino, married, and a

resident of Brgy. Santiago, Concepcion, Tarlac, after having been sworn to in
accordance with law, hereby depose and state –

1. That in my Certificate of Live Birth as Recorded in the Philippine

Statistics Authority (PSA), it is stated that I was born on 26 December 1956 at
Barangay Santiago, Concepcion, Tarlac;

2. That in the records of the Social Security System (SSS), it is stated

that I was born on 25 December 1955 at the same address above-stated;

3. That I am executing this Affidavit of Discrepancy to attest to the

truth of the foregoing facts, it is apparent that I had been an inadvertent
mistaken in the recordal of my date of birth in my SSS, as regards to my date of
birth;

4. Further, I am declaring that from this date hereon I would be using

my true and correct date of birth of 26 December 1956 in all my papers, records
and other documents.
