AFFIDAVIT OF DISCREPANCY
I, LEAH MAY CONTIGA y QUEDING, of legal age, and with address at
Brgy. Nilasin 1st, Pura, Tarlac, under oath, state:

1. THAT, I am the daughter of JOSELITO BAGO CONTIGA;

2. THAT, the name and date of birth of my father was registered in his Certificate
of Live Birth as JOSE BAGO CONTIGA who was born on June 11, 1961.
However, during his lifetime, he had used the name and date of birth JOSELITO
BAGO CONTIGA who was born on June 15, 1961 in all his public dealings and
documents.

A copy of his valid IDs are hereto attached for ready reference;

3. THAT, I personally know that he used the name JOSELITO BAGO CONTIGA
and not JOSE BAGO CONTIGA and he knew that his date of birth was June 15,
1961 instead of June 11, 1961 because I am his daughter and I was with him for
most of his life;

4. THAT the name JOSELITO BAGO CONTIGA who used the date of birth of
June 15, 1961 and JOSE BAGO CONTIGA who registered his date of birth as
June 11, 1961 refer to one and the same person, the father of herein Affiant;

5. THAT I am executing this Affidavit to attest to the truth of the foregoing facts
and to use the same for whatever legal purpose it may serve.
