JOINT-AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, MAE FLOR HINGCO PLANAS, of legal age, Filipino, Single and a resident
of P-Petchay B., Golden Ribbon, South Montilla Boulevard, Butuan Ciy , Agusan del
Norte and RHEZA KRISTINE GUIBONE SY, of legal age, Filipino, Single and a resident
of B-10, L-5, Happy Homes Subdivision, Ambago, Butuan City, Agusan del Norte,
Philippines, being first duly sworn to in accordance with the law, do hereby depose and
state;

1. That we personally knew ELVIE LACERNA DANHONG (Elvie for brevity) who
was born on July 21, 1991 at Langihan, Butuan City, Agusan del Norte thru her
parents named ANITA LACERNA DANHONG (mother) and TEOFILO CAMPUS
DANHONG (father) who tied their knot on April 28, 1959 at the New Apostolic
Church of Calaitan, Bayugan, Agusan del Sur;

2. That we have knowledge of the above narrated facts because of the fact that we
are long time friends and we share common facts about our families;

3. That we know for a fact that Elvie was already baptized but not yet registered in
the Local Civil Registrar’s Office of Butuan City, Agusan del Norte, Philippines for
her parents failed to cause the same due to inadvertence and oversight;

4. That Elvie is not registered in any other Civil Registrar’s Office within the
Philippines;

5. That the Traditional Hilot during the delivery of Elvie was Julia D. Badbad of
Kolambugan, Sibagat, Agusan del Sur.

6. That we executed this affidavit to support the application for delayed registration
with the office concerned and to show proof of all the foregoing facts above
narrated and for whatever legal purpose it may serve best.

IN WITNESS WHEREOF, we hereunto set our hands this 16th day of February
