AFFIDAVIT OF LEGITIMATION

We, Ramir Maningo Formentera and Ma. Charmaine Saldua Guevarra, both of

legal age, Filipinos, and residents of Mantalongon, Dalaguete, Cebu, after having been duly

sworn to in accordance with law, do hereby depose and say:

1. That we are the biological parents of Christian James Guevarra Formentera

who was born on December 21, 2012 at Bulacao Talisay City Cebu.

2. That at the time of the conception and birth of said child we were not yet married although
neither of us suffers any legal impediment to contract a valid marriage;
3. That at the time of the conception of the child, neither one of us was under eighteen
(18) years of age;
4. That our child was not conceived and born out of an adulterous, incestuous or
bigamous marriage, or of a marriage void by reason of public policy;
5. That on May 17, 2018 ,we subsequently entered into a valid contract of marriage at

Mantalongon, Dalaguete, Cebu, the marriage contract of which was registered at the Civil

Registrar’s Office of Dalaguete, Cebu under Registry Number 2018-151;

6. That is to the best interest of our child to be conferred with legitimated status;
7. That we execute this affidavit to establish the foregoing facts for the purpose of conferring a

legitimated status to Christian James Guevarra Formentera.
