AFFIDAVIT OF LOSS
(RE: OFFICIAL RECEIPTS FROM ALSC)

I, LUCILA LAROCO, of legal age, Filipino citizen and with residential

address at NIA Road, Brgy. Pinahan, Quezon City, after having been sworn to in
accordance with law hereby depose and state;

1. That I am the purchaser of a the residential house and lot from

Asian Land Strategies Corp. located at Block 72 / Lot 2 and Block 2,
Lot 7, of Dreamcrest Homes Subd., Phase 5-C, Longos, Malolos
City;

2. That payments in installment basis were made thru me who kept

the OFFICIAL RECEIPT Nos. 480425, 492404 and 501425 for the
lot payments and OR Nos. 472669, A55492, 480424 and 492402 for
the house payments exclusive for Block 72, Lot 2, Dreamcrest
Homes 5-C, Longos, Malolos City;

3. That lately however, when I was looking for said OR’s at my files at
home, the same was misplaced/lost and nowhere to be found
including OFFICIAL RECEIPT No. 412830 for the payment made
for lot located Block 2, Lot 7, Dreamcrest Homes 5-C, Longos,
Malolos City;

4. That despite diligent efforts to locate the said Official Receipts

proved futile.

5. That I am executing this affidavit in order to attest to the truth of

the foregoing facts and circumstances behind the loss of the said
OR’s from Asian Land Strategies Corp. and for whatever legal
purpose this may serve.

FURTHER AFFIANT SAYETH NAUGHT.
