AFFIDAVIT

I, MA. CRISTINA B. ZAMORA, of legal age, Filipino and residing at Zenia St.
Elvinda Village, City of San Pedro Laguna, after having duly sworn to according to law
hereby depose and say:

1. That, I am the attorney in fact of RAMONITO ABAGAR RELIQUIAS whose

name appears on the certificates of employment, job trainings, and association
affiliation issued by his previous employer and association/society in which he is
bonafide member;

2. That, said certificate of employment, trainings and association affiliation

certificates were legally obtained and secured by the herein affiant;

3. That, these certificates/pertinent documents are in need to be authenticated by

lawful authorities as valid requirements for traveling abroad;
