AFFIDAVIT OF LOSS

I, CESAR C. GECALE JR., of legal age, Filipino, married and residing at Blk 2 Lot 57 Phase
11, Carmona Estate, Brgy. Lantic Carmona Cavite, after having duly sworn in accordance with
law, hereby depose and state that:

1. That I am a holder and owner of TRAINING CERTIFICATE - ISM/”K”LINE

QUALITY AND SAFETY MANAGEMENT SYSTEM in the year 2020.

2. That sometime in December 2022, while looking for the above TRAINING
CERTIFICATE - ISM/”K”LINE QUALITY AND SAFETY MANAGEMENT
SYSTEM the same was missing and must have been lost when I moved out from my
parents’ house. That I loss the said documents without my knowledge.

3. That I exerted diligent efforts to locate the TRAINING CERTIFICATE, but efforts
remain futile, and I therefore considered the same as lost and can no longer be found.

4. THAT I am executing this Affidavit to attest to the truth of the foregoing facts and to
use the same for whatever legal purpose it may serve.
