MOTION FOR EXTENSION OF PROVISIONAL

AUTHORITY

COMES NOW, the applicant , SHRINE CITY CABLE ANTENNA

TELEVISION, INC. through undersigned counsel, and unto this Honorable

Commission, most respectfully alleges that;

1. Applicant is grantee of a Provisional Authority (PA) to install, operate

and maintain a Cable Television (CATV) System in the municipality of
KAUSWAGAN Province of LANAO DEL NORTE on December 9, 2015
valid for a period of eighteen (18) months or until July 9, 2017
Copy of the Order is hereto attached as ANNEX “A”;
2. That applicant has already complied with all the terms and conditions
set forth and enumerated in the said PA;
3. In connection here with, it is respectfully prayed for the Honorable
Commission that the extension of the Provisional Authority be issued
to applicant;
4. The extension of the Provisional Authority is essential to be able to
lend continuously to the service, and thus, benefit their existing
subscribers in the municipality of Kauswagan, Lanao del Norte.

WHEREFORE, premises considered, it is most respectfully prayed for the

Extension of Provisional Authority, in the interest of public service.

Petitioner prays for such other remedies the Honorable Commission may

Deem just and equitable under the circumstances.

Rizal Ave.Ext.Estaka,Dipolog City, Zamboanga del Norte, Philippines, hereby name, appoint, and
authorize my daughter JAIRRAH REALIZA UMPA, Filipino legal age and a resident of Sultan
Palace, Rizal Ave. Ext. Estaka, Dipolog City, Zamboanga del Norte, Philippines to be my true
and lawful attorney in fact to do the following acts:

1. To transact, negotiate, process and pay our loan obligation before the
Community Rural Bank of Dipolog (CRBDI)-Dapitan City Office;
2. To claim and receive the Owners Duplicate Copy of Transfer Certificate
of Title No. T-11291 used as collateral of the said loan obligation before
the CRBDI;
3. To make, execute and sign the necessary documents for the release of
such loan and to submit the release of mortgage before the Register of
Deeds and cancel its annotation;
4. To transact and negotiate with BRAIAN M. PELAYO and/or PELCOM
Trading for the sale and/or rental of our commercial/residential
property situated at Rizal Ave. Ext. Estaka, Dipolog, City duly embraced
under Transfer Certificate of Title No. t-11291 and to surrender to the
Mr. Pelayo the said TCT No. T-11291;
5. To execute, make and sign the necessary and other relevant documents
to fully facilitate the foregoing act and authority afore-mentioned;
6. To do all other acts in conformity with the above-mentioned authority.

HEREBY GIVING AND GRANTING unto my said attorney full powers and authority to do
and perform all ordinary acts and deeds necessary to be done in order to carry into the effect
the foregoing authority to be done and about the premises, as fully for all intents and purposes
as I may regularly and legally do if personally present with full powers of substitution and
recovation, hereby ratifying and confirming all that my said attorney or his substitutes shall
lawfully do my cause to be done premises.
