AFFIDAVIT OF SERVICE

We, FERDINAND CRISTOBAL, of legal age, Filipino, and with address at 1155 5th F
San Benisa, Quirino Highway, Novaliches, Quezon City and WILLIAM TEMPORAL JR., of
legal age, Filipino, and with address at 14 Lucban St., Brgy. Pasong Tamo, Quezon City ,
after having been duly sworn in accordance with law, hereby depose and say that:

1. On April 13, 2019, we served the copy of a DEMAND LETTER dated March 22, 2019
issued by Atty. Paul Dominic R. Velasco to MR. REDDIES SAYO at his office address,
specifically, NO. 108 ANTIPOLO STREET MAYPAJO CALOOCAN CITY;

2. The copy of the said DEMAND LETTER was personally served to, received and
signed by MR. REDDIES SAYO as evidenced by his own signature on the upper right
portion of the receiving copy of the said letter;

3. Attached herewith as Annex A is the copy of the DEMAND LETTER dated March 22,
2019 issued by Atty. Paul Dominic R. Velasco which was personally received and
signed by MR. REDDIES SAYO on April 13, 2019;

4. We are executing this affidavit in a good faith to attest to the truth of the foregoing
statements and for any other legal purpose, it may serve.
