--JOINT AFFIDAVIT--

We, FLORA G. JIMENEZ and GREGORIO P. DADULLA, SR., both of legal age, married, Filipino
citizens and residents of and with postal address at No. 417, Int. North Bay Boulevard (North), Navotas
City, after having been duly sworn according to law, depose and say:

That, we personally know the deceased member, CONSTANCIO SORIA ASIS who died in
December 6, 2012 and the only son of Tripon Asis and Mercedes Soria, since we have been neighbors
since 1981;

That, during his lifetime, he was living with Elena Asis for 29 years and they are blessed with two
children: Joanne Asis born on November 4, 1982 and Johnny Asis born on May 21, 1984;

That, to the best of our knowledge, both parties do not have prior marriages;

That, Elena and the children have been dependent for support upon the member until Constancio
got sick and can no longer support them;

That, Elena decided to work abroad to support the medical and other financial needs of Constancio
and the family;

That, Elena has never remarried and has remained faithful after the death of Constancio in
December 6, 2012;

That, we are executing this joint affidavit to attest to the truth of the foregoing and for all legal intents
and purposes.
