I, Anastasia Uy, of legal age, single, Filipino and resident of Brgy. Cuartero Jaro, Iloilo City after duly sworn to in accordance with the law hereby depose and say, That, I am the sole heir of Andres Uy, being the only legitimate daughter as evidenced by the copy of Certificate of Live Birth, herein attached as Annex A; That, the decedent died intestate with no debts left at Jaro, Iloilo City on August 8, 2010 as evidenced by a copy of Certificate of Death herein attached as Annex B.
