JOINT-AFFIDAVIT OF MEMBER’S NEAREST RELATIVES

WE, GERARDO A. GONZALES and RODOLFO BROSO, both of

legal age, Filipinos, and residents of PNR Site, Legazpi City, and Pigcale,
Legazpi City, respectively, after having been duly sworn to in accordance
with law depose and say:

1. That we personally know VICENTE A. BROSO because he is our

cousin;

2. That we also know the fact that his common-law spouse is

LUZVIMINDA B. MORADA and they have one child named JESUS M.
BROSO who was born on September 26, 19__;

4. That LUZVIMINDA B. MORADA is not separated with VICENTE

A. BROSO;

5. That LUZVIMINDA B. MORADA was not dependent for support

upon VICENTE A. BROSO;

6. That LUZVIMINDA B. MORADA did not remarry nor cohabited

with other man neither she has children with other man prior to the death
of VICENTE A. BROSO.

7. That we also know the fact that VICENTE A. BROSO has no other
spouse or common law spouse.
8. That we also know the fact that he has no illegitimate child from
other woman.
9. That we are executing this affidavit to attest to the truth of the
foregoing statements and for whatever legal purpose this may serve.
