JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS
We, ANITA L. ALFORQUE, and EMILIANO SEDON both of legal
ages, married and residents of Brgy. Mahayay Digos, Digos City, davao
del Sur, Philippines, after having been duly sworn to in accordance with
law, do hereby jointly deposes and says:
That we are the same and identical persons who caused for the
execution of this affidavit;
That we personally know the name person of MAMERTO
ALBARRACIN, who was born on August 18, 1953 at Brgy, Mahayahay,
Digos City, Davao del Sur as reflected in his Baptismal Certificate and
Marriage Contract;
That the fact of his birth was not registered at the Local Civil
Registrar of Digos City, Davao del Sur, due to negligence and
inadvertence;
That until at present the said fact of birth of the above-named
person was not yet registered in any Local Civil Registrar;
That we are not related by blood nor by affinity or consanguity of
the above persons;
That we are executing this affidavit in order to attest the truth of
the foregoing facts and in order to apprise the proper authorities of the
same and for whatever legal purposes it may serve;
Further affiants sayeth naught.
