AFFIDAVIT OF CO-OWNERSHIP

I, RONNEL VICTOR C. LUCE, of legal age, Filipino citizen, married, and with
residence address at 648 Bahay Pangarap, Brgy. San Roque, Sto. Tomas City, Province of
Batangas, after having been duly sworn to in accordance with law do hereby depose and state:
1. That I am one of the co-owners of the business “O’CUP” located at _____________;
2. That my name was not registered in the above-named entity but it was duly registered
with the Department of Trade and Industry up to present;
3. That I am an applicant at National Housing Authority for House Transfer of Rights for
Blk 1 Ext Lot 2, Barangay Santa Fe, Cavite City;
4. As such, I was required to declare my monthly income;
5. I am earning approximately ONE HUNDRED TWENTY THOUSAND PESOS
(PHP120,000.00) which derives from the above-mentioned business;
6. Through this Affidavit, I hereby declare under oath that I am co-owner of “O’CUP” and
that I am earning ONE HUNDRED TWENTY THOUSAND PESOS (PHP120,000.00)
which derives from the above-mentioned business;
7. I am executing this Affidavit to attest to the truth and veracity of the foregoing facts.
