AFFIDAVIT

I, SAMUEL ALEXANDER ASPREC PASIA of legal age, Filipino, and a resident of Phase 3 B3 L11 Macadamia
Street, Greenwoods Executive Village, Pasig City, Philippines after having been duly sworn to in
accordance with law, hereby depose and state, that:

I am the Incorporator of AVASIA INFORMATION SYSTEMS, INC. registered corporation with SEC No.
CS201814915 with the Securities and Exchange Commission.

I confirm that the correction made specifically the Tax Identification Number 724-027-899-000
appearing in the duly submitted duly signed and notarized hard copies of the company’s Articles of
Incorporation and By-Laws of AVASIA INFORMATION SYSTEMS, INC. were made prior to notarization
have errors and were made with the full knowledge and consent of all the other incorporators.

I hereby present the correct Tax Identification Number 616-892-817-000 to be applied on the
Company’s Articles of Incorporation and By-Laws.

I assume full responsibility for the above-mentioned corrections and wishes your office considerations
to correct the committed error and hold all staff of SEC-CRMD free from criminal, civil and
administrative liabilities, in relation thereto;
