office address at Yangco Law Office Unit 1102-A, West Tower Philippine
Stock Exchange Center (Tektite) Exchange Road Ortigas Center, Pasig City
, after having been duly sworn in accordance with law, hereby depose and
state that:

I am a member of IBP Rizal, San Juan, Mandaluyong Chapter (RSM)

with Roll of Attorneys No. 71649;

1. I passed the Bar on April 26, 2018 In compliance of A. M. No. 17-

03-09-SC otherwise known as the Rule on Community Legal Aid
Service (Rule on CLAS), which requires new lawyers to render one
hundred twenty (120 hours) of pro bono legal aid services to
qualified litigants within one (1) year from the time of their signing
of the Roll of Attorneys.

2. I have rendered the required one hundred twenty hours of pro bono
legal aid services in IBP RSM (Rizal) Chapter Legal Aid Program
, as evidenced by the attached authenticated Time Sheet, a copy of
which is herewith attached as “Annex A”;

3. I am executing this affidavit to attest to the truth of my full

compliance and for all legal intents and purposes it may serve.
