AFFIDAVIT OF CHANGE OF NAME ARTEMIO S. MAGNAYE

I, ARTEMIO S. MAGNAYE, of legal age, Filipino, and with a residential address at

3718 Bautista St., Brgy. Palanan, Makati City, after having been duly sworn to in accordance
with the law, do hereby depose and state:

That I am a representative of Federal Land Inc., with Building Permit No. 01-08784 and
issued on June 6, 2019;

That, based on the record of our Occupancy Permit issued on January 5, 2021, the
registration name appeared as EDWARD LOPEZ JR., Unfortunately, I would like to manifest
that EDWARD is no longer connected to Federal Land Inc.;

That I hereby, attest to the truth of the foregoing facts and request to change his name on
our Occupancy Permit to proper authorities concern;

That I am executing the foregoing instrument in order to attest to the truth of the above
facts for all legal intents and purposes it may serve.
