AFFIDAVIT TO RECOVERY OF LOST TITLE

We, SPS EDWIN D. MAGPANTAY AND MARIE ANCILLA F. MAGPANTAY, both

of legal age, Filipinos, married, formerly residents of 792 F. CRUZ STREET,
ZEPHYR HOMES, MALIBAY, PASAY CITY, and current residents of 30 MANILA
BAY DRIVE, SOUTHBAY GARDENS, PARANAQUE CITY, having been sworn in
accordance with law, do hereby declare that:

1. That, we are the registered owners of a condominium unit in SEA Residences

identified and described in Condominium Certificate of Title (CCT) No. 003-
2015-008667, as:

Phase 3 – Building E
Floor Level – 16th Floor
Unit No. – 11
Description – 1 Bedroom Unit
Unit Area – 24.00 sqm

2. That we executed an Affidavit of Loss in relation to CCT No. 003-2015-

008667 on 20 May 2019 before the Atty. Carlos M. Caringal, Notary Public,
with Doc. No. 438, Page No. 89, Book No. XXVI, Series of 2019;

3. Said Affidavit of Loss was annotated in the record of the said title in the
Registry of Deeds (RD) for the City of Pasay under Entry No. 2019003525
dated 22 May 2019;

4. Rebecca Velasquez Bermudez, sister of Elvira Velasquez Aban, found the

lost copy of the original title in her residence last 30 August 2019 and
surrendered to Affiant the said copy of the original title;

5. We hereby certify that the statements made herein are true and correct to the
best of my knowledge and belief.
6. We executed this Affidavit to attest to the truthfulness of the contents of this
document and for the purpose of requesting the Office of the RD for the City
of Pasay to annotate this Affidavit.
