This letter serves as my authorization to Rudelyn Acop as my representative to request the waiver of

interest charges totaling P3,392.30 included in the Statement of Account (SOA) for Unit 1624 D as of

November 13, 2019, a copy of which accompanies this letter.

As discussed over telecom yesterday with representatives of your office, I did not receive notices for the

association dues because they were sent to an incorrect email address. As agreed, I have updated my

personal information (including the addition of an alternative email address) to ensure that I receive the

notices and able to pay them on time.

In this regard, please accept the enclosed payment amounting to P34,337.45 representing my total dues

as of the attached SOA, excluding interest charges.
