Cabahug St.,Centro,Mandaue City,Cebu,Philippines after having been duly sworn to in
accordance with the law,hereby depose and state:

1.That I am the owner of CAROLINA'S FOOD CORNER with business address at 104 S.B
Cabahug St.,Centro,Mandaue City, Cebu,Philippines and registered under the Business Permit
Division of Mandaue City respectively;

2. That the said business with address 104 S.B Cabahug St., Centro,Mandaue
City,Cebu,Philippines has no business operation March 202-September 2021;the reason of the
said business of the above-mentioned company was due to COVID19 pandemic;

3.That I am executing this affidavit for the purpose of establishing the veracity of the foregoing
facts and circumstances in compliance with the requirements of PENCON Application, in
connection with my business closure to terminate my taxes and for whatever legal intents and
purposes this affidavit may serve best.
