AFFIDAVIT
I, BHERNARD ANIEL M. DE LEON, of legal age, Filipino, single, and with
address at 8684 4th St., Sto. Niño, Parañaque City, after having been sworn in accordance
with law do hereby depose and say:

1. That sometime on March 2016, I registered online my taxpayer’s

identification number (TIN) and given the TIN number 326-488-842-0000.

2. That however, when I filled up the registration form online, I chose

“professional” as my employment status instead of “local employee.”

3. That I am updating my employment status from “professional” to “local

employee.”

4. That I am executing this affidavit to attest to the truth of the foregoing and
for the purpose of updating my records in the Bureau of Internal Revenue and for
whatever legal purposes this may serve.
