AFFIDAVIT

I, CRISTY PAULINE GARCIA FLORES, of legal age, single, Filipino

citizen and a resident of Moncada, Tarlac after having been duly sworn to in
accordance with law, do hereby depose and say:

1. That I am the registered owner of a business with name “PINK’POSH

SALON AND SPA”, located in Moncada, Tarlac;

2. That I hired MARY ANNE CARBONEL, on March 10, 2018, in the said
business as hair stylist;

3. That I pay for the monthly Social Security System (SSS) contribution
of MARY ANNE CARBONEL, from March 2018 up to December 2018;

4. That MARY ANNE CARBONEL was employed in my business and only

reported for work until January 8, 2019 without any notice to me as
her employer;

5. That the abovementioned employee was only employed in my

business from March 10, 2018 until January 8, 2019 and the
termination of her employment was not immediately reported with
Social Security System (SSS);

6. That I am now attesting that MARY ANNE CARBONEL is currently not

employed and affiliated in my abovementioned business;

7. That I am executing this Affidavit to attest to the truth of the foregoing

statements and for whatever legal purpose this may serve.

in the said business as manicurist;

3. That I pay for the monthly Social Security System (SSS) contribution
of OWEN BALIGNASAY PARSAMENTO, from March 2018 up to
December 2018;

4. That OWEN BALIGNASAY PARSAMENTO, was employed in my

business and filed his resignation letter informing that his last day of
employment is until December 30, 2018 (Copy of his resignation letter
is hereto attached and marked as Annex “A”);

5. That the abovementioned employee was only employed in my

business from March 10, 2018 until December 30, 2018, and the
termination of his employment was not immediately reported with
Social Security System (SSS);

6. That I am now attesting that OWEN BALIGNASAY PARSAMENTO is

currently not employed and affiliated in my abovementioned business;

7. That I am executing this Affidavit to attest to the truth of the foregoing

statements and for whatever legal purpose this may serve.
