after having been duly sworn to in accordance with law, under oath, do hereby depose and state:

THAT our wedding took place on October 20, 2018 at 5:00 in the afternoon before REV.
LUIS ANTONIO S. SEIJO at BALAI YLLANA, BF RESORT, Talon Dos Las Pinas City.
Attached herewith is the certification attesting to such fact.

1. THAT we are executing this Affidavit to attest to the truth of foregoing facts of our own
personal knowledge that our wedding took place at BALAI YLLANA, BF RESORT,
Talon Dos Las Pinas City on October 20, 2018. This is for the purposes of registering
the said marriage with the Office of the Civil Registrar General of LAS PINAS CITY and
for whatever legal purposes it may serve.
