AFFIDAVIT OF LOSS

I,  FELICIANO A. DE GUZMAN of 106 Labao St. Ligid Tipas Taguig City,
Filipino, at legal age, after having been duly sworn in accordance with law, hereby
depose and state:

1. That I am the official representative and signatory of the business

ALTON METALCRAFT BUILDERS located at 106 Labao St. Ligid
Tipas Taguig City.

2. That it’s been a few weeks now that we are looking for the ORIGINAL
CERTIFICATE OF REGISTRATION (COR) of ALTON
METALCRAFT BUILDERS issued by Bureau of Internal Revenue
(BIR) that I discovered that it was missing;

3. That despite diligent search and efforts to locate the said ORIGINAL
CERTIFICATE OF REGISTRATION (COR), we could not find it, such that,
I now believed that it was lost, and is now beyond recovery.

4. That I am executing this statement to attest to all the above facts.
