PHILOS FACIAL AND BODY CLINIC located at Lipa City, Batangas;

2. That I have been issued a DTI PERMIT No. 02169585 valid from
September 16, 2013 to 16 September 2018;

3. That on January 31, 2018, I shall close the said business due to change of
business;

4. That I execute the foregoing instrument in order to attest the truth of the
above facts for all legal intents and purposes it may serve.
