AFFIDAVIT OF TWO DIS-INTERESTED PERSON

(For Late/Delayed Registration of Death)
We, REMEDIOS B. TACIO, of legal age, married, Filipino Citizen with residence and
postal address at Datakan, Kapangan, Benguet, Philippines, and ESTHER B. BOCAWE, of
legal age, married, Filipino Citizen and resident of Cabanalian, Datakan, Kapangan, Benguet,
Philippines, after having been duly sworn to in accordance with law hereby depose and state that:

1. We are residents of Datakan, Kapangan, Benguet and among our neighbors is the late
MARTIN CAMBIS BALINGIT, Jr.;

2. Said person died single and without any children at his residential house at Datakan,
Kapangan, Benguet;

3. On April 21, 2022, we were passing through the residential house of Mr. Balingit when we
smelled foul smell coming from the said house. We decided to check what was that foul
smell and went inside the house because it was not locked and we were surprised to see Mr.
Balingit lying on the floor and on a decomposing state or maggots already in his entire
body;

4. We informed Ms. AUREA C. FAGEL regarding what happened to her brother Martin Jr.
and on the same date, April 21, 2022, Ms. Fagel came. Mr. Martin Jr. Was buried near his
house on April 22, 2022;

5. Unfortunately though, the facts of death of Mr. Martin Jr. was not immediately registered
with the Local Civil Registrar of the Municipality of Kapangan, Benguet due to the fact
that the said office was closed at that time due to Covid-19 Quarantine until such time that
the registration was delayed;

6. Now Ms. Fagel, intends to register the facts of the death of her late brother Mr. Martin Jr.
through the late registration process;

7. That we know the facts of the death of the aforementioned decedent as we were among
those who help during the wake and burial and the fact that we are neighbors of the
decedent;

8. We are not in any way related to the above-mentioned family neither by consanguinity nor
affinity;

9. We are executing this Affidavit to attest to the truth of all the foregoing facts and
circumstances and to support the Late or Delayed Registration of Death of MARTIN
CAMBIS BALINGIT, Jr., and for all legal intents and purposes that it may serve.
