AFFIDAVIT OF GUARDIANSHIP

I, ROSITA M. BARGASO, a senior citizen, Filipino, widow, and a

resident of Dalisay, Gumatdong, Itogon, Benguet, after having been duly
sworn to in accordance with law, hereby depose and state;

1. That I am the guardian-in-fact of the two minors, namely: JAN

XAVIER P. BARGASO and MARK HAILEY C. BERNARDEZ, the
former being the child of my son Jesus Bargazo with Racqueliza
Bargaso and that the latter being my grand granddaughter she
being the child of my granddaughter Timmy Lisa Bernardez, also a
widow;

2. That both of the above-named minors live under my custody -

their parents being OFWs working abroad;

3. That as the guardian-in-fact of the above-named minors, I

exercise control and supervision over their person and welfare to
the best of their interest;

4. That I am executing this Affidavit to attest to the truthfulness of

the foregoing and for all legal intents and purposes.
