AFFIDAVIT OF LOSS
I, VANESSA GAYO FERNANDEZ, of legal age, single, Filipino Citizen
with residence and postal address at 470 Maria Pucay Extension, Pinsao
Proper, Baguio City, Philippines, after having been duly sworn to in accordance
with law do hereby deposes and states that:

1. I am an owner of an under armour Black Bag where I kept my Blue

Pouch with money, Red Envelope with BIR Book and Official Receipts;

2. On October 22, 2022, I lost my aforesaid bag, I rode a taxi going to

Pinsao, Baguio City. Upon arriving home, I was not able to notice that
my bag was left inside the said taxi. Only after an hour when I realized
that my bag was missing, unfortunately I was not able to get the Plate
Number of the said taxi;

3. Despite of my exerted diligent effort to find my said bag, still I failed,

hence, it is considered forever lost;

4. The said armour Black Bag, Blue Pouch with money, Red Envelope
with BIR Book and Official Receipts, were not given nor pledged to
anyone for any purpose whatsoever, nor were taken or confiscated by the
authorities;

5. I am executing this affidavit to attest to the truthfulness of the foregoing

facts and for all legal intents and purposes that it may serve.
