AFFIDAVIT OF CONFORMITY
I, TERESITA P. DERAS, widow, of legal age, Filipino Citizen and resident
of Tubongan, Kayapa, Nueva Vizcaya, Philippines after having been duly sworn
to in accordance with law hereby deposes and states, that:

1. I am an owner of a certain parcel of land located at Tubongan, Kayapa,

Nueva Vizcaya with an area of 19,920 Square Meters, covered and
embraced by TRANSFER CERTIFICATE OF TITLE No. T-48086;

2. I engaged the services of Geodetic Engineer Ferdinand M. Reyes to

subdivide my aforesaid parcel of land which he did and after conducting
the subdivision survey on the ground, he prepared the necessary
subdivision survey on October 30, 2022;

3. Prior to the aforesaid subdivision survey, there were two subdivision

surveys that were conducted and these two (2) previous subdivision
surveys are now consolidated and subdivided into Seven (7) Lots;

4. I hereby confirm and affirm the consolidation and subdivision of the lot for
its approval and issuance of separate titles thereafter for easy
maintenance of taxes;

5. There is no conflict or protest on the corners and boundary descriptions of

the prepared, executed and submitted subdivision plan;

6. I am executing this Affidavit to attest to the truth of all the foregoing facts
and to support the approval of the subdivision to be submitted, and for all
legal intents and purposes that it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 18th day of
