AFFIDAVIT OF LOSS

I, IVY KAYE, of legal age, Filipino, single and a resident of Mabini Homesite,
Cabanatuan City, Nueva Ecija, after having been duly sworn to in accordance with law,
hereby deposes and states THAT:

1. I am the safe-keeper of Transfer Certificate of Title No. XXXXX registered

in the name of my auntie Julie G. Viloria, covering a parcel of land situated
at Brgy. Pesa, Bongabon, Nueva Ecija with an area of TWENTY SIX
THOUSAND NINETY TWO (26,092) square meters;

2. The Owner’s Copy of Transfer Certificate of Title No. 2012000035 was

entrusted to me by the registered owner for safe-keeping since the time of
its issuance and was kept in an envelope placed inside a cabinet in my
residence together with other important belongings;

3. Sometime on September 15, 2020, I noticed that the envelope containing

the certificate of title was already missing from my files and personal
belongings;

4. Said certificate of title is nowhere to be found and despite exerting diligent

efforts to locate it, everything proved futile. Hence, I now consider the said
TCT as legally lost and beyond recovery;

5. The property covered by Transfer Certificate of Title No. XXXXX has not
been mortgaged, pledged nor delivered to any person, private entity or
banking/ financial institution for any credit or obligation whatsoever;

6. I am executing this affidavit to attest to all the above facts, to request the
Honorable Register of Deeds for Nueva Ecija to annotate at the back of the
original copy of the aforesaid TCT the fact of loss of the owner’s copy
thereof and in order to file a Petition for Issuance of Second Owner’s Copy
of the Transfer Certificate of Title No. XXXX.
