sworn to in accordance with law hereby deposes and states, that:

1. I am the business owner of a certain business which is a

2. I was forced to closed and stopped the operation of the said business
due to financial reverses or bankruptcy since January, 2023.
However, I was not able to process for the formal closure of the said
business with the pertinent government offices or concern. Hence, I
am asking for apology;

3. It is only now that I am processing the formal closure and

surrendering of the business permit and other pertinent documents of
my said business for the main reason that, it is only now that I have
enough financial resources to pay the closure fees and other
surcharges/penalties that accrued and whatever legal fees;

4. I hereby undertake that same scenario/situation will not happen

again in the near future;

5. I am executing this Affidavit to attest to the truth of the foregoing

facts and to support the formal closure of my aforesaid business, and
for all legal intents and purposes it may serve.
