I, __________________________________, of legal age, Filipino and with postal address at
________________________________________________, after having been duly sworn to in accordance
with law, hereby depose and state that:

1. I am the Corporate Secretary of ____________________________________, (herein the


“Corporation”) a domestic corporation with office address mentioned above;

2. The Corporation upon its incorporation have initiated and started


feasibility studies and assessments of potential mining sites;

3. Thereafter, it filed its Exploration Permit Application with the Mines and
Geosciences Bureau;

4. Presently, said application is still on process hence the Corporation is yet


to commence its commercial operation;

5. The Corporation hereby undertakes to commence commercial operation


once its Exploration Permit application is approved and issued;

6. I am executing this Affidavit to attest to the truth of the foregoing facts and
for whatever legal purpose/s it may serve.
