We are pleased to confirm that after careful consideration, [Name of the Company] (herein “the
company”), has decided to extend you an offer of employment. The details of your offer are as follows:

Your position with the Company will be [POSITION], and you will be expected to perform the duties and
responsibilities required of your position as described in the job description. Your immediate supervisor
will be [NAME AND POSITION]. This position is FULL-TIME, and you will be required to work for
[NUMBER OF HOURS] hours per week. Should you choose to accept this position, your start date will
be the date you first signed your employment contract (i.e., [DATE OF ORIGINAL EMPLOYMENT])
with your previous employer, [Name of previous employer]. (herein “________”). The Company
reserves the right to change your hours and duties as it deems necessary.

Your compensation will be [BASIC MONTHLY SALARY] per month, with the proper deductions made
for all required withholdings. Company payroll runs on a bimonthly (5 th and 20th day of the month) basis.
You will receive Company benefits such as paid vacations and health coverage. A full description of
these benefits is hereto attached (the “benefits”). In addition, you will be compensated for overtime in
accordance with Company overtime policy and Labor Laws. The Company reserves the right to change
your compensation and benefits as it deems necessary.

As an employee, you will be expected to abide by Company rules and policies. An Employee Handbook
is available at the Human Resources Department for your reference. You are expected to read the
handbook and acknowledge your understanding in writing upon receipt of your copy. You may not
disclose confidential Company information to unauthorized third parties, and at no time you may disclose
confidential information of a former employer to the Company. A copy of the Confidentiality and Non-
Disclosure Agreement with Non-Compete Clause (herein “the agreement”) is hereto attached and must be
signed and returned to the Company once completed.

Your primary place of work will be in [Work Location]. You agree and commit to be transferred,
seconded, or assigned to other locations, subsidiary, or associate companies in the Philippines. In
addition, you likewise agree and commit to work at the company’s place of business or other areas such
as field work or work outside the company premises, as your immediate supervisor or the Management
may prescribe now or in the future.

This letter, the attached agreement, compensation package and benefits, attached annex, including the
Employee Handbook form the complete and exclusive statement of employment between you and the
Company. These employment terms supersede any other agreements, understandings, promises, or
communications, written or oral, by or on behalf of your former employer.

If you haven’t completed yet the submission of your pre-employment requirements, please comply as
soon as possible. The submission of the following valid documents for employment, such as, but not
limited to, (a) Medical Certificate, Proof of Residence (i.e. passport, or driver’s License, or voter’s ID
card); (b) NBI (National Bureau of Investigation) Clearance; (c) Proof of Previous Employment (i.e.
Certificate of Employment from last employer); (d) Proof of SSS Number (E-1/E-4 Form); (e) Proof of
PAG-IBIG Number; (f) Proof of Tax Identification Number (TIN) and (f) tax compliance (i.e. BIR Form
1902 or 2305, and BIR Form 2316); (f) Photocopy of birth certificate; (g) Photocopy of marriage
contract if applicable; (h) Photocopy of birth certificate of dependents; (i) Proof of education (i.e.
transcript of records or diploma); (j) four (4) pieces 2x2 ID photo; and (g) four pieces 1x1 ID photo are
necessary as these were the pre-condition of your pre-employment.

In the event of unsatisfactory fulfillment of the pre-employment terms and conditions, the company
reserves the right to cancel this Contract.
