AFFIDAVIT OF EXPLANATION

I, ___________________, of legal age, married, Filipino Citizen, with

residence and postal address at _________________________, Philippines, after
having been duly sworn to in accordance with law hereby deposes and states,
that:

1. I am the proprietor/owner of _______________________

CONSTRUCTION with business address at ______________________;

2. The said business started to operate sometime in January, 2008 but

it did not start well, as it was very hard to sway clients/customers. No
one is engaging my construction firm or in short no transactions;

3. Although I registered with the Pag-IBIG and was issued Employer ID

No.: 2042 4208 004, I was not able to pay the Pag-IBIG contribution
because of difficulty in gaining income from my said business, instead
of gaining income, I instead gained loss;

4. Only sometime in June, 2013, that I was able to pay the Pag-IBIG
contribution because atleast clients are now engaging my
construction firm for project;

5. The aforesaid circumstance explains the reason why only in June,

2013 that I was able to pay my 1 st contribution with Pag-IBIG. The
non-payment from January, 2008 to May, 2013 was not advertently
done, but only due to the aforesaid circumstance;

6. I am executing this Affidavit to attest to the truth of the foregoing

facts and for all legal intents and purposes it may serve.
