postal address at _____________________, Philippines after having been duly
sworn to in accordance with law hereby deposes and states, that:

1. I am engaged in business, particularly a Sari-Sari Store located at

_____________________________;

2. My business is located along the __________________ Road and without

space for parking purposes;

3. Most of the clients of the aforesaid business are walk-ins or

passers-by, barely are customers who have motorcycle or motor
vehicle. I hereby undertake that the customers who have
motorcycles or motor vehicles shall find any parking space
nearby before going to my store to avoid traffic congestion;

4. I am executing this affidavit to attest to the veracity and truthfulness

of all the foregoing facts to support my application for issuance of
Business Permit for my business, and for all legal intents and
purposes it may serve.
