AFFIDAVIT OF NON EARNINGS

 
I, _______________________, of legal age, Filipino citizen, and with
business address at _______________________________, Philippines, after
having been sworn in accordance with law, hereby depose and state that:
 
1. I am the proprietor of ________________________________ registered and
permitted to engage in business with line of business in
____________________________________________;
 
2. My business was registered at the office of the Social Security System
and as such I was religiously paying the monthly contributions of my
employees;

3. However, on _________________________ upon the declaration of the

Enhanced Community Quarantine due to the COVID-19 pandemic I
voluntarily stopped the operation of the said business;

4. I categorically state that since March 17, 2020 up to May 16, 2020,
my business establishment did not operate. Further, my employees
have not reported for work hence, I cannot pay their monthly
contributions since my business has not generated any income in
whatever form;
 
5. I am executing this affidavit to attest to the truthfulness of the
foregoing facts and whatever purpose this may serve.
