JOINT-AFFIDAVIT

WE, JUVY N. BANILLA and SANDY B. BAWANG, Filipinos, both of legal

age, and residents of Bacolod City, Philippines, after having sworn to in
accordance with law hereby depose and state:

1. That we personally know the person of MAY RICCIEL RUBRICO Y

MISAJON, who was born on May 10, 1986 in Parañaque to parents
Sally Misajon and Juan Rubrico, because we were neighbors with her
family when they transferred in Bacolod City;

2. That the said MAY RICCIEL RUBRICO y MISAJON was born on May
10, 1986, as indicated in her Certificate of Live Birth;

3. However, in her Marriage Certificate, May Ricciel Rubrico’s birthdate is

written as May 05, 1986;

4. We attest that the correct birthdate of May Ricciel Rubrico y Misajon is

the one that is indicated on her Birth Certificate, and that the person
indicated therein refers to one and the same person who was born to
the same parents abovementioned and who is also the same person
indicated in Marriage Certificate Registry No. 2014-1636;

5. We are not in any way related to the said MAY RICCIEL RUBRICO y
MISAJON, nor her parents or any member of her family; and

6. We attest to the truthfulness of the foregoing.
