SSS SICKNESS BENEFIT

This is to certify that Mr. RICKY B. CAUBE has been a regular employee of VMP CDO GROUP OF
PROPERTIES CORP. since October 2019. His position was Maintenance/ Driver and his Job
description was to Keeping the company vehicle clean and properly maintained by performing regular
washing, Safely transporting company staff as well as various products and materials to and from
specified locations in a timely manner, Assisting with the loading and offloading of products, and
materials, Carrying out vehicle maintenance checks.

He has properly separated last february 2023, Mr. Ricky B. Caube did not receive any advance
payment of his SSS Sickness Benefit from October 2022 during the day of his sickness occured until
present. The Company was able to pay his contribution until february 2023 to be able to assist with his
condition (Half Stroke).

This certification is issued to Mr. Ricky B. Caube this 5 th of March 2023 for the purpose of complying
with the documentary requirements of the SSS for his sickness benefit claim.
