Re: Cancellation and Refund of BenLife Insurance Contribution

Greetings!

This letter is a formal request to cancel my monthly BenLife contribution amounting to

Php 1,000 effective January 2024.

I would also like to request the refund of my total contributions from October 2022 to
December 2023 amounting to Php 15,000. My reason for the cancellation and refund of
my contribution is due to my ongoing application to a housing loan to provide my family
stability, security and a better life. My salary as a Patrolman cannot afford to pay my
monthly contribution in this insurance because I also availed the AFPSLAI and PSSLAI
Capital Contribution Account (CapCon).
Attached below is my personal details together with my valid I.D for your reference.
