AFFIDAVIT OF LOAN DENIAL

I, RADZHATA JAILANI ABDURASAD, of legal age, widow, Filipino and
permanently residing at Fronting SULECO, Bus Bus St., Bus Bus, Jolo, Sulu,
Philippines, after having been duly sworn to in accordance with law hereby depose and
say that:

1. I am formerly employed as Administrative Officer IV of Parang National High

School, ARMM. As such, I am covered by the Government Service Insurance
System (GSIS).

2. My application for Retirement under RA 8291 was duly approved by the

Government Service Insurance System (GSIS).

3. However, in the course of processing my Retirement, I was informed that I

allegedly obtained an Enhanced Salary Loan (ESL) on March 7, 2002 for the
amount of Php 114, 576.00.

4. I hereby vehemently and unequivocally deny under oath and in good faith that I
obtained the aforesaid Enhanced Salary Loan (ESL) with the Government
Service Insurance System (GSIS). I vehemently and unequivocally deny
involvement in the aforementioned Enhanced Salary Loan (ESL).

5. I hereby execute this affidavit under oath and in good faith to attest to the truth
the foregoing facts based on personal knowledge and authentic records and for
whatever legal purposes it may serve.

IN WITNESS WHEREOF, I hereby sign this Affidavit this 18th day of November
