AFFIDAVIT OF UNDERTAKING
I, ALYANNA SAIRA OMAR KANTI, of legal age, Filipino citizen and a
resident of 19 Monserat Drive, Baliwasan Road, Zamboanga City, Philippines,
after having been duly sworn to in accordance with law hereby depose and say
that;

1. I am the lawful and absolute owner of a MITSUBISHI STRADA GLS

2.4L with plate no. MAD9770, which was involved in an incident that
transpired on November 13, 2022 at intersection Corcuera St. and NS
Valderosa St., Zamboanga City;

2. The above-described motorcycle was taken into custody by the Police

Station No. 11, Zamboanga City Police Office, because of the said incident;

3. To this date, the said vehicle is still in the custody and in possession
of Police Station 11;

4. That said vehicle is not among those articles or instrument or tools

which are considered illegal per se. Hence, it is most respectfully requested
that said vehicle be released;

5. That upon release of the said vehicle, I will undertake to produce said
vehicle at any time that the Police Station or the Honorable Office of the
Prosecutor or the Honorable Court would necessitate or require me to do so;

6. I am executing this Affidavit under oath and in good faith to attest to

the veracity of the foregoing facts in support of the release of the above-
described motor vehicle from Police Station 11 and for whatever legal purposes
and intents it may serve;

IN WITNESS WHEREOF, I have hereunto set my hand on this 16th day of

November 2022 in the City of Zamboanga, Philippines.
