AFFIDAVIT OF WAIVER OF RIGHTS

I, Mr. REYNALDO ALINTEJO CANDELARIO JR. Filipino, of legal age, single and a resident of
Purok Malipayon, Kablacan, Municipality of Maasim, Sarangani Province after being duly sworn to,
hereby depose and state as follows:

(1) That I am the same person involved in a motor-vehicular accident that transpired on
October 13, 2023 at about 9' oclock in the morning along National Road, Brgy Kablacan, Maasim,
wherein I was driving a Motorstar 150 with plate number _____ that was bumped by a Mitsubishi
Strada with plate number SKX 899, the latter being owned by Department of Agriculture RFO XII and
driven by its employee, Mr. Ernesto Grecia;

(2) That after a careful and thorough investigation of the circumstances of the accident, I am
fully convinced that the mishap was purely accidental for which the said Department of Agriculture
XII or its agent could not be held responsible;

(3) That in view of the circumstances, I am no longer interested in, and hereby desist from
the further prosecution of the said driver and owner criminally or civilly after arriving to a settlement
to cover the actual and compensatory damages for the loss or inury incurred as a result thereof
amounting to TWO HUNDRED THOUSAND PESOS (PhP 200,000.00); and that in case a criminal
action has been already filed against him, I hereby request the authorities concerned to have the
said case dropped and/or dismissed;

(4) That I have read the foregoing and/or the foregoing has been explained to me and
translated in Tagalog, Visayan dialect which I understand, and I hereby affirm/confirm the same to
be true and correct.
