I, ALEXCES BILLONES MENDOZA, a Filipino, of legal age, Single and a resident of
Purok Bonifacio, Brgy. Guinsang-An, Santo Niño, South Cotabato, Philippines, after being
sworn to in accordance with law, depose and state:

That I am the registered owner of a certain motor vehicle more particularly described as

was caught by a Land Transportation Officer for violation of Driving an Unregistered Motor
Vehicle;

2. That I hereby undertake that I have already registered the above-described motor vehicle to
comply with the rules and regulations of the Land Transportation Office;

3. That I am making these statements under oath and in good faith in order to establish the fact
that the said motor vehicle was already registered;

4. That I am executing this affidavit voluntarily to attest to the truth of the foregoing facts based
on personal knowledge and authentic records in compliance with the requirements of the
Land Transportation Office and for whatever legal purpose it may serve best.
