COMPLAINT AFFIDAVIT

I, am ______________, of legal age, married, and with postal address

________________________, after having been duly sworn to in accordance with law do
hereby depose and say;

That, on _____________, at around 2:07 in the afternoon, an unidentified number

called my cellphone. The male caller identified himself as an employee of BPI (Bank of the
Philippine Islands) and soon thereafter, he made me talk to his female manager. The manager
introduced herself as “Shaira”. Shaira asked for my account number, username, birth date and
the one time password that was forwarded to my cellphone. I acceded to her requests and
gave her the details she requested. Immediately thereafter, I was no longer able to access my
online BPI account, a problem that I was able to raise with Shaira. Shaira gave me a temporary
password that enabled me to open my BPI account temporarily. Upon opening my account, I
noticed that the “savings” button was now labelled as “cashback” button. Shaira explained that
cashback points would bring money to my account and she then instructed me to change the
limit of the instapay in may account to ___________. I complied as instructed. Thereafter, she
asked me to transfer cashback points to cash bank account number “1616045413" number but
the transfer failed. She then asked me to transfer the cashback points to cash bank account
number “8239653079” but the transfer also failed. She then instructed me to transfer the
cashback points to the account that was identified as “Shaira Dela Cru, A/C#8239653079”.
When I transferred the cashback points as instructed, P90,000 from my savings account was
transferred to the said account. It was then that I called BPI Festive Mall branch.

That I attest that the foregoing statements and the submitted supporting documents are
true and correct to the best of my knowledge and belief and I commit myself to be liable for
perjury and/or dishonesty as a result of any fraud, false or misrepresentation in this affidavit.
