PERSONALLY came and appeared before me, the undersigned Notary, the within named Annie
Ghuznavi, who is a resident of Silver Spring, State of Maryland, and makes this statement and
General Affidavit upon oath and affirmation of belief and personal knowledge that the following
matters, facts and things set forth are true and correct to the best of their knowledge.

I first filed on June 6, 2021 because my contract with Weber Solutions did not renew. The
BEACON system was not able to recognize my earnings, so they could not make a
determination. I had to reach out to my state delegate to get any correspondence. The
DOL told me that they needed my proof of earnings, taxes, identity, and proof of
separation. I uploaded all the documentation that was required and they have confirmed
that they received it. When I was able to contact a DOL representative, they do not
understand why my case has not been processed.

I have not been able to file the weekly certification since I submitted my original claim.
On July 11, 2021, per DOL instructions, I reopened my claim via email. Even after a long
time on the phone with a representative, eventually I was hung up on. I asked my state
delegate to help me inquire again. I inquired with the DOL inquiry email about why I
need to reopen my claim, but the response I received was that I simply needed to do it,
and to call back in two weeks. To this date I have received no unemployment benefits.

The average total of my monthly bills is about $2000 per month. I’m behind on my
mortgage and other bills. It’s also hindering me from looking for another job. I was not
able to file for taxes this year because I knew I would not be able to pay what I owe. It
also has an effect on taking care of my 4-year-old. If I cannot receive unemployment
benefits, I cannot afford to fix my car, buy work clothes, or any of the necessary tasks to
have a successful job hunt. My husband is also having problems with Unemployment
Insurance so our household cannot depend on either of our income.

I first filed in November 2019 because my contract with Marriott Hotels came to an end.
I received regular state unemployment insurance until it ran out in March 2020. In
November 2020, DOL sent me an overpayment letter, saying there were three separate
overpayments, and that I owed them in total about $33,000.

I was able to contact a DOL representative who was able to correct one of three of the
overpayment errors. Later, in email correspondence, DOL representatives let me know
that the other two overpayments were incorrect and that they would submit a ticket
(Maryland UI Request: 00533700). To this date the issue has gone unresolved. Because
of these unresolved issues, DOL is docking money from my current unemployment
benefit payments.

The average total of my monthly bills is about $2000 per month. I’m behind on my

I was working at the Amazon Broening Highway facility. Before that I was working at a
restaurant in Hampden. I was forced to leave Amazon because so many people became
ill with COVID. I applied through the Beacon system in November 2020. I received full
unemployment at first but then my profile was turned off. I reapplied and received $100
dollars for approximately two months. After President Biden signed the covid aid bill, I
received the 300 dollars approximately two times. After that my Bank of America debit
card was closed. I was told I needed to confirm my identity, so I submitted my drivers
license, birth certificate, and social security card twice. I am locked out of my account on
fraud allegations. The only person I was able to talk to was a contract worker in Arizona,
not an actual adjudicator.

The situation has pushed me into a state of depression. I was supposed to move back to
Seattle or somewhere in the pacific northwest. I couldn’t afford rent at my apartment so I
had to leave. I spent the small amount of unemployment I did get earlier on bills and
food. I have not been able to afford the mental health care that I need. Since I could no
longer afford my apartment, I have been living out of my car and couch surfing since last
year.

When the MDOL made the weekly certification process operable, I was able to get that
first week pushed through. The following week I reapplied on June 13, 2021, wanting to
avoid any setbacks. After I reapplied, I was kicked out of the PUA program and my
account was wiped out and converted to Regular UI. Ever since then, I haven't received
any payments, and three if not four of my weekly claims went from on hold to
disqualified. My last two are currently on hold. I was able to speak to a few agents, and
two of them were on the same page while the others didn't know what was going on. One
out of the two agents who was in the know stated that she didn't know why MDOL was
telling people to reapply. She also stated that my account would convert back to PUA
once someone gets to my claim because it wasn't supposed to convert from PUA to
Regular UI at that time. It was evidently done prematurely.

The second guy whom I spoke with pretty much said the same thing, and from his end he
could see my PUA account including the missed payments along with every document
that I've previously provided to be eligible. He also stated that what probably happened
was by reapplying, the algorithm of the Beacon Portal read as if I was applying for the
first time on top of the system being already glitched. The other agents were clueless.
Furthermore, he also informed me that he was putting in a ticket, and escalating it to the
highest tier, requesting they release my funds and convert my account back to its original
state because all of my documents and information was in the system, readily available,
and nothing was out of sync for an unnecessary adjudication to take place. I do believe
besides the portal being glitched that we were prematurely kicked out of the federal
programs intentionally.

knowledge.

On February 9, 2021, I was terminated from my job at H&S Distribution for a safety
violation. I applied for unemployment benefits the next day. I set up my account on
BEACON shortly thereafter. After not hearing back for a month, I called the DOL every
day. When I finally reached someone, the DOL representative told me that my employer
was not contesting my claim but they could not find my identity verification. The
representative told me that my case would be flagged and not penalized for not having
identity verification because my BEACON account never prompted me to upload it. The
representative sent me the prompt while I was on the phone, and waited on the phone
with me so that she could confirm that she received it and that it looked okay.

A month and a half later, I managed to contact a DOL representative, who told me that
my case was pending adjudication. They could not give me a date when my case would
be adjudicated and to wait for correspondence.

About three weeks later, an action item appeared on my BEACON account that I had to
send in my identity verification. I sent this again. I received correspondence that my
verification was received. Another action item appeared and I had to answer questions
that appeared to me as the kinds of questions that would be asked during adjudication. I
sent my answers in. After another three or four weeks, I inquired with the DOL about the
status of my case. The DOL representative told me that she did not see any reason why
my case would be on hold. She found in my file that I was supposed to receive my funds
within three business days of June 17, and if I did not receive anything, to reach out to the
DOL. When I reached out, a different DOL representative told me that they did not know
what to tell me and that I was simply pending adjudication.

To this date, I have received no unemployment benefits. I am late on my car payments, I


have not paid my rent, I cannot pay my car insurance, and the IRS cancelled my payment
plan. I have had to have hard conversations with my children that I am in a difficult spot
and I cannot help them with their needs. The average total of my monthly bills is about

$1300. If I cannot start paying on my IRS payment plan, I am at threat of having my


property repossessed.

on July 4, 2021. I have been filing my weekly certifications through the webcert portal on
Beacon. I have not received any benefits. I have kept calling but no one from the Labor
Department has gotten back to me. I also check my Beacon account every day. My
Beacon claim number is #202101.

I have no money at all to pay my bills which includes my rent, utilities, car insurance,
phone bill, food and other important necessities. I live alone and have no one to help me
with my expenses.

I was working At Sam’s Club but was laid off because of COVID. Bus lines restricted
service and getting to my job in Severn was no longer possible because buses were not
running properly. I loved that job. I had worked there for six years. In October of 2020,
I applied through the Beacon system. I have not received any money. I was approved at
first. However, when the unemployment Bank of America card came to my house, it was
closed. I was told I needed to provide proof of identity and income. I sent in W-2s and
my drivers license twice and I also mailed hard copies of my W-2s and drivers licence.
My account was then locked for being “under fraud investigation.”

I have not received a single dime of unemployment insurance or pandemic benefits which
I qualified for since October, 2020 to today’s date, July 20, 2021.

This has caused me horrific stress. I am behind on bills including water and electricity. I
have had trouble paying for gas, food and other basic necessities. I struggle with mental
health issues and they worsened significantly. I went without insurance for a long time. I
need prescription medication and struggle to afford that for a long time.

I was with the same valet company Towne Park from May 2015 to March 2020. I
initially was receiving full unemployment after I got laid off because of COVID. I was
put on hold in early May, 2021. Eventually, I was told I was being investigated for fraud.
Since June 6, 2021, I have been on hold for fraud adjudication. I have not been able to
get in touch with anyone to get more information about adjudication. I get a different
answer every time I talk to someone. I was told it was resolved but it never was. My
account is now locked and I cannot access my information. I have not received benefits
since the end of May 10, 2021. The Labor Department owes me from May 10, 2021 to
July 9, 2021.

My lease expires in August and I am running out of money. I am not sure where I am
going to live. All this has devastated me financially. I hereby swear that the above

knowledge.

I was a school bus attendant for 28 years when I was laid off in March of 2020. I filed for
unemployment immediately. I received benefits until February of 2021. When I attempted to
access my BEACON account, I was locked out pending identity verification. I was asked to send
various identity documents into the Department of Labor. I sent in my drivers license and my
social security card, front and back.

I sent in those documents seven or eight times. Every time I sent in my documents I was told that
the documents were not received or there was an issue with what I sent. This ranged from “not
showing all four corners of the document” to an allegedly unsigned social security card.
Eventually, my account was flagged as potential fraud. I could no longer log into my BEACON
account to file. I have not been allowed to file and I have not received any payments since
February.

I am a current cancer patient. This is my second bout with breast cancer; however, this cancer is
more aggressive and dangerous. Medicaid is the only reason I have been able to stay afloat. I
need this money to survive while I fight this awful disease. I have been unable to pay my rent,
my phone bill, and my car payment. This has added tremendous stress, which is not ideal for a
cancer patient. I worked for almost 30 years serving the school system of Anne Arundel County.
I believe I have earned the right to live as comfortably as possible. I need this money.
