AFFIDAVIT-complaint

I, MICHAEL PONCORDAS, of legal age, married and a resident of Phase IV,

Salansang Village, Barangay San Isidro, General Santos City, after having been duly
sworn in accordance with law, hereby depose and state that:

1. I am an investor of ACT OF CHARITY CONSUMER GOODS TRADING

whose business address located at Fred-Cion Building , J. Catolico Barangay
Lagao, General Santos City;

2. This business entity is a legitimate business center engaged in Trading and

Investment. And is covered legally to operate with the 2019 Mayors Permit
number 05092-1 and Business Identification Number B17320190102062 and
Department of Trade and Industry Certificate Number 05648043 and Bureau
of Internal Revenue Certificate of Registration. All registered in the name of
Proprietor Business owner Pastor BERNIE NARCISO BAUTISTA;

3. Sometime in February 15, 2019 I invested a modest amount in this company

as I believe it would generate extra income for my family and I have friends
also who have earlier invested and got the interest they have been looking
for. Upon filing my investment form the name of Pastor BENIE BAUTISTA
was prominently displayed in the form as the owner;

4. True enough after the lock-in period of four months in my investment, I was
able to harvest a substantial amount of Thirty Seven Percent of my
investment and some of my friends who knew of my Pay-out also approached
me and wanted to join and invest. I did invited them but out of their own
volition and their own free will voluntarily joined the investment fever so to
speak;

5. Sometime in August this year 2019 as more of my relatives, friends and

acquaintances joined in under my referral, problems in the issue of Pay-outs
surfaced. This was brought about of the closure of some investment entities
similar to this company, The ACT OF CHARITY CONSUMER GOODS
TRADING can no longer sustain the operation of having Pay-outs to those
who have invested more specifically my own group of friends whom I helped
to join;

6. Since this problem has tremendously affected me personally including my

family and emotionally because of the complaints leveled against me by my
circle of friends who have trusted their investment to me. Night and Day and
wee hours in the morning my cellphone is ringing to get their money back
and some instances make threats to my life;

7. Personally I has invested under the name of my wife MARILOU ENAD

PONCARDAS a very huge amount of almost One Million Five Hundred Pesos
(Php1,500,000.00) and this part of our life savings and other jewelries we
pawned to augment the capital investment. And this will really break my
heart if I cannot recover this amount from Pastor BAUTISTA;

8. Since I have been pushed to the corner I cannot promised them anything
except to file a formal complaint against Pastor BERNIE BAUTISTA for him to
bail me out of the mess he has done. And be held answerable and
accountable;
 9. I am executing this affidavit in order to apprised the proper authorities of the
foregoing and to help me support the formal complaint against Pastor
BERNIE NARCISO BAUTISTA.
